Discovery Scope (Written Protocol Only)

Service Description

You will provide your full health details and personal goal(s). We will provide a 14 week written Protocol Review. Consultation is not included. Clients receive one 10 minute checkin. We highly recommend scheduling a checkin so that we receive your completed Discovery Scope form 48 hours before the checkin appointment. Make this valuable time more efficient by being prepared for this check-in appointment. To prepare, please have questions or concerns ready. Also, if you are a caretaker, please have proper full knowledge of the client's health condition(s). If you have more than 1 - 2 questions, please schedule a "Follow-Up Session" on our Book Online web-page. If you decide to include a 90 minute consultation, you must schedule it within 30 days of sending your Discovery Scope to our office. Within this time-frame, the cost difference will be charged for the bundle. After 30 days, the fee of $175 for a 1-hour consult is applied.
